Minor: With a Point of Divergence far enough back in time to prevent the fall of Carthage, I don't think most, if any of these other states would come into existence.

The Carthaginian Empire is a remnant of Southern Roman Empire. It's a Christian state.

The "lakes" in Ukraine (or Sultanate of Hungary on this map) are actually Soviet-era damming projects.

Do the outlines around some countries signify anything in particular?

I used here a blank map of wikipedia's style.

The Outlined states were formerly in long-time confederation and their relations are still strong. Poland,Lithuania and Little Russia were within personal union under one Gediminid king. The Greater Russia and Chrobatia were ruled by a minor lines of the same dynasty.
