According to the story they are in, it comes with a variety of side effects, notably infertility, constant pain and torment, mental retardation. Every transformation is literally burning yourself from inside out then regenerate, and the humanoid form is pretty ugly too. Cursed dragon shape-shifters are incredibly violent and always raging. Thus they are unable to communicate, take care of themselves or perform complicate tasks, and their intellectual capability is just slightly more than that of apes. As a result, to be useful they must be brainwashed and put under domination spells. The curse can only be place on an infant, if the subject is just slightly older such as a toddler, it's 100% fatal.

This is medieval setting so a punishment to kill off the sinners' bloodline quite fit the theme though
