Description My interpretation of SCP-049, the Plague Doctor.

For my design, I wanted to incorporate an organic appearance to his cloak, with the occasional vein here and there. In addition, I gave his cloak stitches and fake seams; these are not a part of his form, but a cosmetic feature that 049 sewn onto himself to give the illusion of fabric. Though upon closer inspection, the illusion is quickly broken when you notice that his gown is covered in hairs and pores. He would also likely smell rather foul, since he rarely bathes, and lots of stuff such as blood and viscera tends to get stuck in the folds of his "cloak" during his operations. The mask grows right out of his facial bones, and the flesh of his face is fused to it, making it less of a mask and more of a "second face" so to speak. His nasal passages are also connected to the "nostrils" of the mask, giving his voice a slight nasal sound. The only part of his original face that has not fully fused with the keratin mask is his mouth, which allows him to speak. His mask even has a jaw joint, allowing him to eat food if it is offered to him despite not requiring sustenance (the article states that he has a liking for thin crackers, salted pork and hard cheese), though his actual mouth is not visible from the outside. Due to his nasal passages being drastically elongated from the mask, his voice and breath has a resonating, metallic sound to it. As stated in the article, his underlying skeletal structure is humanoid, but I also thought it would be an interesting idea if his skeleton was altered to a small degree as well. His skeleton is like that of a human male, though his arms are strangely long, his shoulders are rather broad, and his build is a strange mix of slim and muscular.

Personally I like to interpret SCP-049 as once being human, and was probably quite a rational and intelligent man. Though his intentions are good, they are highly misguided, and his descent into madness was one of many factors that caused him to slowly become the maddened creature that he is today. His discovery and study of "the cure" would have also contributed to the corruption of his body.

Every time I remember that his cloths are actually just skin I get grossed out. I really hope it at least feels like fabric and not skin.

That doesn't seem to be the case. They don't die of brain aneurysms, they just die. He has to inject various substances and apply electricity for them to become zombies.

i can think the instant-killing touch is probably powered by necrotic energy from some unknown source, possibly primordial deities called The Brothers Death as shown in the Death and the Doctors Hub, the Plague Doctor is said to be a member of the Children of the Barrow, but considering his humanoid appearance he may have been human before becoming one of them.

That's actually very interesting. I haven't heard of the Brothers of Death until now, and it kind of makes sense how 049 would be connected to them. I reread the 049 article's footnotes, and one of the footnotes linked to the hub you mentioned. There's also another footnote linking to the Hanged King hub, so he probably has some connection to the Hanged King as well.
