Yeah on real people it's usually about one hand of distance. However, that distance is a matter of artistic choice much like any length on the body.
Bokuman is a cartoonist, he'll aim for "rounded" designs.

It doesn't look rounded so much as he added extra fat where it doesn't go in an attempt to make them look bigger. I'm into stylizing figures as well so I know it is possible to make the proportions rounder and more cartoony without making it too disproportionate.
