megalomaniac, and at the end, almost completely insane, triggering the Age of Apostasy that nearly tore the Imperium apart.

Really great piece. Professional quality in my opinion and it's awesome that you are still looking for critique.
My critique is all about directing the viewer's attention and being a little more dramatic. Right now the light areas are all about the same size and kind of scattered all over. Most of my attention is on the floating skull on the left so I find myself looking at it the most. I might try lightening the guy's flesh to be really pale to try to pull attention to him more. I'd maybe lighten his hair to be very white as well to try to get attention more to his face. I think even a kind of spotlight on the main character would add some drama to the piece. You have candles everywhere no strong rim light. I think adding some to the tubes behind him and the sides of the hat and head will help to round those forms and put him in the environment. The floating skull just to the right of the chair could be catching some stronger light from below and to the left from the candle. It's hard to tell where that skull is in space without seeing it overlap or catching some more obvious lighting cues. The only cue to where it is in space is by comparing it's size to the size of the main character's head. The hanging wrist pendant distracts me a little as it's pretty light on a pure black background and it doesn't seem to be hanging but is swinging. I would make that hanging straight down and lower it's contrast some since it's not very important.
The extremely dark background is flattening things out a bit for me. You have some windows back there creating back lighting and candles as well but it's still pitch black. If there's any sort of air in there much less dusty or smokey air then it can be a lot lighter. You could even add some halos of light around the candle flames.
I don't have a lot of drawing critiques really. The forearm with the upturned hand seems particularly long. The other forearm seems particularly long but feels referenced so I suspect the folds where that arm bends are a little off and it's making the arm seem longer.

Just curious why the one servo-skull has the Inquisition Insignia. The Ordos were holding quite back during that time, because they were battling each other if they should support or kill Vandire, and only a small faction was actively taking part in the Age of Apostasy - by finding and supporting Sebastian Thor.
