Yeah in the book, during that famous battle of Sodden Hill she and a bunch of other mages gave it their all to push back Nilfgaard but she was struck by powerful magic and as a result her breasts were almost mutilated. She did recieve basically magical plastic surgery but some scarring was too severe and as a result she became self concious and never wore plunging necklines.
