I don't get what was supposed to have happened with the Exploration Log 1499-D part of this SCP.

to make a long story less long, the gas mask the guy's wearing is the SCP that transports the wearer to this weird alternate dimension where those eyes and mouths creatures exist. I don't want to say any more, for fear of spoiling what happens at the end of this particular moment, but I do recommend you check it out
