I'm a tad confused as to if and when deasupra and dedesubt are used with article or not.
deasupra/dedesubt = on top/bottom
de deasupra de dedesubt = away from the top/bottom?
Now which word would be better: tulpina or mijlocul?

Well.
Deasupra/dedesubt means on top (above)/under
De deasupra/de dedesubt means from above/from below.
To explain it better... the 'de' ones are used in sentences where you could replace the "de" with a "which is/was"
I got the book (which is) on top of that pile = Am luat cartea de deasupra maldarului acela. (couldn't think of a better translation for 'pile', I think 'maldar' is pretty ok though)
Also, dedesubt is a bit different. The usual way to say "under" is "sub", so if you want to say "I'm under the bed" you will say "Sunt sub pat".
Dedesubt (unarticulated) is usually used when that particular sentence doesn't explicitly name the object that something would be under, or at least when you don't have that object after the 'dedesubt'. Example... hmm... "The second she saw the bed, the dog hid under it" = "In clipa in care a vazut patul, cainele s-a ascuns dedesubt."
"What are you wearing underneath?" = Ce porti dedesubt? (it also means this)
"See that pile of files? Yes. Mine is the one way down there." = "Vezi maldarul ala de dosare? Da. Al meu este dedesubt de tot"
Dedesubtul (articulated) is also used quite often. You find it in the same sentences I gave examples for just before, but if you add a noun after them.
"The second she saw the bed, the dog hid under the bed" = "In clipa in care a vazut patul, cainele s-a ascuns dedesubtul patului."
"What are you wearing underneath your clothes?" - "Ce porti dedesubtul hainelor?"
"See that pile of files? Yes. Mine is the one way under the other ones." = "Vezi maldarul ala de dosare? Da. Al meu este dedesubtul celorlalte"
As for tulpina/mijlocul... better use mijlocul. Tulpina means strictly the stem of a plant, meaning from where it goes out of the ground to where the flower starts.
I don't think the 'o at the end' thing was refering to a verb. Hmm.

Those are correct.
Yeah, "l-am salvat" and "a-l proteja". They are different because "l-am salvat" means "I saved him" (past tense) while "a-l proteja" means "(to) protect him". Different tenses.
