When it's not specified, it usually from the hand or deck. There are cards that don't tell you where to get them from.

there is no card in the game with an effect that doesn't state where it special summons a monster or itself from
now the effect is much clearer and easier to understand

Sorry about all the confusion. I would swear I've seen cards that can special summon monsters, but didn't happen to mention where they are being special summoned from.
