The lower jaw is what makes it work for me. The lack of teeth, the jutting chin, and the almost unatural looking lip. Serious mindfuckery. Couple this with the pointed fox face and the hulking androgeny of the thing and you my friend have got nightmare fuel.
The legs made of grotesque agonized souls are nice touch too.
