Let's take a step back... On December 26, 2008, Madara had kidnapped Konan, in order to make Itachi rush to her rescue, so as to kill his nephew without problems. In the previous weeks, Itachi had become problematic for Madara: the young Uchiha was more than ever determined to oppose him, also killing Obito to take revenge on the rape that the latter had implemented against Konan and, not least, Itachi had fallen in love with Konan, promising her to free her from that hell, even at the cost of life. In order to put him to the test and continue to deprive him of people dear to him, Madara comes to a decision: Itachi will no longer be his worthy heir, but his younger brother Sasuke. Madara orders Sasuke to kill his parents and, finally, Sasuke should have killed Itachi. Sasuke, partly galvanized by the false promises of Madara and accented by hatred for his brother, performs what was ordered, but for a series of circumstances he can not kill Itachi, dying in his place and exhales the last breath confessing to the older brother who had loved him. With the death of Sasuke, the pivot of the story, the fulcrum of his sacrifice, Itachi understands that, despite having managed to free the city from Madara and be back as a free man, the pain and guilt for not being able to understand everything and save his family from Madara's madness it brought Itachi to break up with Konan, despite knowing that she needed him and vice versa, choosing a kind of forced exile in another city.
Three and a half years later, Madara is informed of recent events. He didn't know that Itachi had returned to Tokyo, nor that he had returned with Konan or that the two had children; moreover, his empire, his family (a part of it, at least), patiently waited for a sign of his rebirth, and Izuna tries to convince him to start everything again, in any way.
Therefore, what is understood from the page is Madara's surprise for having known certain things, which he will use (?) to his advantage. Maybe...

Would Madara have any advantage if, by chance, the children were his own and not Itachi's?
Undoubtedly, yes. Despite being in prison and no longer being one of the many bosses of the Yakuza, Madara has discrete power over his acolytes (the most fanaticals) and it would not be difficult to hire someone and threaten Konan and then marry her also against her will, as he had decided many years ago, when he went to get her at the orphanage and then make her a prostitute.
If Itachi had not come back to live with her and the children, Madara would have killed Konan without thinking twice, and he would take the twins to the orphanage (he hates children, and he doesn't like the idea of becoming a father, for some reasons).
But since Itachi is present on the Tokyo scene, now Madara has an extra interest to complete his revenge...

Is there any correlation between Itachi, Konan and Madara?
Yes, theirs is a kind of love triangle. Itachi and Madara are in love with Konan, even if for different reasons...
- Itachi loves Konan because, despite the slow, gradual and difficult growth of their relationship, first as roommates and then as close friends, she had accepted him for the kind of man he was (and that he continues to be), without ever refusing him for the person he had been forced to become
- Madara loves (or he loved?) Konan only for purposes related to the clan. For Madara, Konan would become his woman-object, a "wife" in name but not in fact.
