oh heheh, well that was just a gift for my pal who is meant to be in the picture, I didn't really make a story for that, just the idea of being a hostage came up hahah. hmm lets see:
1)lineart layer
2)colouring and details layers underneath - may be in several parts
3)lighting layers above all of these and play around with 'overlay' on these, it is quite useful.
sometimes I ref or use some pictures from online & manipulate them, but in this case I just imagined it^^ and yes usually I have to think of one first.

aww just keep trying and don't worry too much, many artists don't use backgrounds. The concept stage is the most time consuming, colouring is the fun part IMO^^
