Looks great. Really like the idea.
But do you think that you could make a new one with the spelling corrected? As mentioned before, "immediately" and "other" isn't spelled corrected. And I'm not sure, but I think that the long sentence is... wrong, somehow. Shouldn't it be "... and activities are to be immediately reported..."? I'm just asking, not complaining.
Again, it looks great. Will definitely save this if it's corrected.

Check your spelling on 'immediately', too. This may be just my own thing, but I find that misspellings detract from the impact of a piece. Good job, otherwise.
