the details in this photo are fantastic - like the texture of the tinsel, the tiny capillaries in the eyes, the glossy sparkle hidden in the lips. It all seems a very surreal work with a lot going on, yet with a strong focus (the direction of the eyes). The bloodwork is does not seem overtly realistic to my eyes -- but I must stress that I think that is NOT a negative point - there is an undefinable purity about it.

I think the only true point I could criticise would be that you have omitted the use of the yellow filter/shader you have used on the hand - and therefore this sticks out as a bit of a white "sore thumb" (sorry for the pun - i couldn't avoid it ), as your composition is based on black, yellow red with a dash of green. i think that colouring this to match to yellower colour of the face would strengthen the continuity of the piece no end.
