I read somewhere that on the book 4 dvd commentary bryke said that firelord Izumi has a daughter that Mako was supposed to end up with, so I decided to draw what I thought she would look like, kinda wish we got to see her!Related content

To be fair Iroh II was the first born, so HE would be firelord. The Fire nation is safe from the horror that is mako...on a throne.

however upon retrospect Iroh II is as of book four 40yo and there is no indication of him being married and/or having kids, so they might possibly give the throne to his sister.

however upon further retrospect Izumi herself is the Firelord and her husband is never mentioned or even made to seem important. So if the throne did go to her daughter, SHE would be the ruler not him.
So it would be Fire lord (Insert whatever her name is here) and fire....lady(?) Mako.
