looks nice. Only the shadows in the form fields, i thing, don't fits the design.

How did you add a shadow to the text fields? I've never seen that done before with CSS.

its actually pretty easy... this is just a mockup made in PS, but you can use CSS to make a background image for text fields...

Looks good. It is simple. Maybe a different font as that doesn't work well with making it "warm"
