Isugi actually sits on the floor, with his neck bleeding. Though, instead of the blood he sees the sky full of stars. The sky is supposted to be above him, so he sees it above, and he stretches his arm upwards. In reality, however, he just touches the blood on the floor.
Hallucinating is typical of him, I suppose. I tried to combine the reality and his own illusion here.
