I envisioned that the SgtMaj of the Army in the NCR doesn't necessarily conform to the modern US army where they're the longest serving enlisted/non commissioned member sort of thing and a non-combatant, whereas in the NCR I decided since the rank didn't exist in New Vegas I could make up my own idea surrounding it, which is that the Sergeant Major of the Army is a unique title and rank given to the soldier with the best performance, discipline and service record in the enlisted troops of the NCR Army. And since 'The Courier' in New Vegas saves the NCR from defeat and does the work of about 100 soldiers throughout various quests, it seemed only natural they'd promote him.
