Have you tried using the "Displace" filter in Photoshop to create effects like this?
(Filter-Distort-Displace)
I used it here [link] on the dress and the hand.

Thats an interesting method, I never even knew about that filter. I suppose with brushes you get more control over what you do? I don't know though I've never tried that filter.

There is a certain amount of "trial & error" with the displace filter but it is a lot quicker and can produce some great results. It all depends on the image you base the displace filter on. Check online for a tutorial if you want to give it go or I could drop you a note detailing how to do it.
