This is the Kuchisake-onna of Japanese folklore. Her mouth was sliced open by a jealous lover so that no one would find her beautiful again. Now she wanders the streets asking the living if they think she's beautiful and slashing open their mouths or chopping off their heads if they answer incorrectly.

Fun times.
