This is the same as my last setup, except for a few tweaks and a new wallpaper.<3 it!!

Rom: CyanogenMod 12.1 (Android 5.1.1)
Kernel: Elemental X
Launcher: Nova Launcher 4.1.0-beta2
Theme: Melior UI
Widget: Icon Container made with Zooper Widget, and the clock its a tweaked version of :iconPixelHearts: clock .
Icons: OxygenOS icons .
Wallpaper: FlatWallpapers wall.
Misc.: Xposed + Flat Style Colored Bars to change the statusbar icon colors.

And I use LMT (it's PIE) as a replacement for the soft keys.
To disable the "on screen" soft keys, you need to edit build.prop, add some lines and voilá.
