It is often described in the books that Sirius looks so different after being in Azkaban that Harry can only see traces of the man that he sees in his parents wedding picture. Sirius’s eyes are often the key element here and play a large part in his slow progression back to his old self after escaping. Although I’m not sure this is the way Rowling wrote the book, I always envisioned Sirius looking much more clean cut and normal while living in Grimmauld Place (although living here does bring out old and new demons for him). I imagined his hair shorter(about shoulder length or shorter), and basically back to his pre-Azkaban, self except for his eyes and a new depression that feeling useless and being locked up brings.

This model sheet, like the other one, represents the extreme. Basically in Harry’s third year, this is what Sirius looked like. Come 4th and 5th yr, his appearance will begin to change back into the other model sheet.
