Are you sure? I mean yeah the outline is done in pen but I don't think that kind of shading could be done with a pen, unless they took the extra time to make the details of a pencil stroke using a pen

Yes, it in fact can. Different pens write differently. A pen was most definitely used for most of it, if not all of it.

Actually, when I sketch with pen in my sketchbook, I often find the most ideal pen for shading is a really crappy one. XD But yeah, there is already the chance that she enhanced it in Photoshop or something.
