I'm afraid I might be going to play the part of the half-informed apologist, because it's been a while since I read the manga, and the last time I kept up to date on it was some two or three months past. Correct me if I'm getting anything wrong now.
The pain Sasuke causes Naruto - are we talking about Sasuke refusing to come back to Konoha and Naruto tormenting himself over failing to keep his promise to Sakura? Because I think that for Sasuke, it has less to do with Naruto and more with evading whatever Konoha's policy of dealing with traitors is. But I can still get you there.
The Karin incident, however... It seems like a lot of people got pissed at Sasuke for that. But Karin was in charge of one of Orochimaru's prisons when Sasuke first recruited her for Team Snake, right? And looking up on Narutopedia, "She assisted Orochimaru in his experiments". What I'm getting at is that Karin is very unlikely to be a good person, and more probably has done plenty to deserve a knife in the back. Or a lightning through the chest. And Sasuke would be in a position to know in either case.
Of course, by "the Karin incident" you may also be referring to his treatment of the other pink-headed medic in love with Sasuke - telling her to kill a complete stranger, then trying to kill her. I can get behind his decision to kill Sakura, because she was seriously considering (or trying to convince herself to) killing Sasuke, and in that situation you can't expect more from anyone than to try and kill the other one first. It's the fair and safe thing to do. About making Sakura kill a helpless stranger - well, she had just said she would help Sasuke destroy Konoha. And Sakura has to be able to see that this stranger is a ninja like herself, who would likely not hesitate to cut Sakura's throat if the tables were turned.
What gets me about that scene is that Sakura doesn't do it. If she had just killed Karin there, maybe Sasuke would trusted her and lowered his guard or something. I don't know, but Sakura seemed to think she could fool him. But instead she chokes or hesitates or something, despite being a ninja, supposedly a trained killer, and despite Karin being an enemy. Why did that happen?
Anyway, I really appreciated your thoughtful response and hope I haven't gotten too many things wrong here.

hmmmβ¦ mostly the pain he causes naruto, i guess. i love naruto so much as a character - all i want to see is naruto kick major tobi-ass and become the hokage someday β and sasuke stands in the way of naruto's dream, constantly causing naruto to suffer from extreme guilt over something that he had no control over: sasuke turning traitor. ...plus, sasuke behaves like an asshole to everyone, and has no respect for human life (remember the karin incident?). so thats where the extreme burning hatred comes from.
(disclaimer: i cant bring myself to like sasuke when i love naruto so much. i can, however, sympathize with him - after all, with naruto, the biggest sasuke fan there is, as the main character, i dont think its possible for any naruto shippuden fan to completely despise sasuke, no matter what horrible things he does).
