Wikipedia says (to the extent you trust it ... ) there was an original scary ending on the Rick Moranis version that tested poorly with audiences. So a "happy" ending was put on. Apparently, the newest DVD of the movie has the original ending as an extra. But it's in black and white. The only known color copy was apparently destroyed in a fire.

The original B&W movie where the musical (and thus the later movie) was based had the scary ending. One of Jack Nicholson's first film roles was playing the man who sees the sadistic dentist (played in the musical by Bill Murray)...
