I personally think that ssj3 is reached from the mastering of ssj1...just like when gohan and goku were staying in SSj mode until it felt neutral,then goku reached ssj3 form due to him being used to this form feeling as natural as base form.Then having ssj2 feel like ssj1.And then he tries going into ssj2 which would ofcoarse be swj3.and vegeta never having tried mastering ssj1 like them,could not turn ssj3.ย

Super Saiyan: To reach this stade, a saiyan need to be more than angry (or goku could reach it in the 22th tournament when Krillin was dead..), and going farther than hate and fury..
Ascended Super Saiyan: Firstly reached by Trunks and Vegeta, it need a big boost of power and meditation to "view" the level. It's more powerful than the ssj stade give muscular mass and use more energy.
Ultra Super Saiyan: Or Super Saiyan 1.5, firstly reached by Trunks, then goku (Vegeta could reach it too but he knew it would be a problem), need to push the assj training more farther to obtain this transformation. It give a bigger muscular mass and an incredible power but it decrease a lot the speed and give quick fatigue. (Broly lssj is just an ussj boosted with Paragus' device)
Mastered Super Saiyan/Full Power Super Saiyan: Counted as a stade but a super saiyan could call him that when he get the same power as Goku/Gohan as a super saiyan 1 at the Cell Game .
Super Saiyan 2: Called like this by Goku, this form was first reached by Gohan with the view of his friends/familly being almost killed by Cell/Cell.jr, it not need rage but need training and a big raise power to reach it (Goku/Vegeta, for Gohan, it was his anger that permit him to have that boost).
Super Saiyan 3: Need an INTENSE training as a Super Saiyan 2 to reach a TREMENDOUS Power and then this form
Super Saiyan 4(GT EXPLANATION IS IMPOSSIBLE! But let's try :'D): (the transformation need a tail) Firstly reached by Goku at the battle of Baby, he almost lost his battle when he saw the Earth wich recive the light of the sun (Brutz), so far, nothing wrong, but we have to notice that the celestial sphere isn't a "moon" (sattelite ofย planet) but the earth itself (dunno if the transformation is partly due to this xd), and seeing he will never see his planet, with all his friends gave him a shock to the heart. His was transforming in an Oozaru (great ape) and in the same time in a super saiyan. After every Oozaru's show... Pan did calm down goku and, with the paradox of the golden oozaru body, when goku is seemed to return to normal, his body became different, powerful, rage. (I particulary found this form extremely badass 
Super Saiyan 5: Rumored by fans, drawn by Toriyama himself (as an extreme what if), Toyble (a japanese mangaka) adapted this idea to create dragon ball AF wich is just after gt (few years after), it appeared to be a grey furred ssj4, with long white hairs and reddish skin (descripted like the ultimatest thing in the universe xd) . It has been adapted by other dragon ball fans like young jijii and malik666
Toyble: Goku ssj5: ?
Young Jijii: Goku ssj5: ?
Gohan ssj5: need the crazyest rage ever seen on dragon ball (german unreal player ?)
Vegeta ssj5: trained with gohan by the old kaioshin to master this form.
Malik666: Rigor ssj5: A different ssj5 than the usual one xd, it looks like a mix of super saiyan hair color and base form color, with longer hairs, no fur but treaks, red tail and a tremendous power than can destroy the user's body. Reached by the way of the craziest rage of Rigor to Vegeta.

We made our own story,characters,powers,background.

Its a comic called "Blood Rite". If you took Blade and mixed it with Sin City, thats the comic. Its done entirely in Black, White, and red. Its about a Vampire Hunter with amnesia, who doesnt know if he's a half-human Vampeal, or a full-blooded Vampire. The main plot involves a group of religious-zealot vampires called The Covenant, the main ruling class Vampires called The Hand, and the resurrection of the Original Vampire called The One True.
