The smile seems sewn into place, as if forcing a smile finally took its turn. (was that the phrase? xD) And was she cutting herself? This is scary but it really holds a lot of meaning that relates to reality.

Lots of smiles in the picture for someone depressed, I dont find I smile at all when I get depressed. The picture really seems to represent insanity or demon posseson more than depresson.

But if you notice, it's her demons, or whatever is praying on her that are smiling, and her own smile is stitched on.

Yeah I suppose thats another interpetation of it. But switched on smiles are more of a mark of insanity as I suggested. Mind you so is suicide. so I geuss it all applies one way or another.
