Its not a golden skull, the description of the commission was more in terms of "remains of a dracolich". I do think its from a published adventure, the Drunken Dragon Inn has been mentioned before so must be a "real" place in a Wotc world somewhere.

Is that the phylactery of Azalin the Lich at their feet - I have drawn a very similar picture with the golden hued skull which is the closing scene from a Ravenloft module From the Shadows.

I don't know the name of the campaign these characters played but the name of the Dracolich is Glizmar apparently, so might be just an artistic coincidence. Or maybe my clients are friends of yours, they like the pose and asked it of me - now that would be really hilarious.

There are many things about this image to admire, but I think that my favorite element is the center figure's armor. Very well done, sir!
I know that D&D specifically allows small characters and indirect combatants to contribute as much to victory as meat-shield-types, but something about the diminutive fellow on the right makes me wonder just what he did to help bring down that dragon. XD

To be honest I don't know what to tell you. The description presented him as a druid 'tho the client really pressed upon the presence of the shield and of its colors. As a druid, his presence is not really required into a melee but we must also consider that he might be a high level character with an animal form (or more).

Ah, I can understand how that would be. He looks like an easygoing, passive fellow, but he may wear a completely different face into combat.
