I used Zbrush, 3ds max, 3d coat and photoshop. For the hair I started with a couple of strands in 3ds max and then moved into zbrush where I did the majority of the work, and the final render is a bunch of different renders from Zbrush BPR post processed/composed in photoshop later.

I've only ever dabbled at Zbrush but I think I should spend more time with it to learn the ropes. It seems like such a versatile program!
