Toyotama was a kunoichi born during the Warring States Period, previous leader of the Uchiha clan and wielder of the Eternal Mangekyo Sharingan. She was the wife of Tajima and the mother of Madara and Izuna. As the leader of the Uchiha clan, she led the group of elite ninja, even her own sons, to battle their rival clan; the Senju, in order to gain supremacy of the land they lived. Due to the opposing views of both clans since their respective ancestry, they constantly fight, risking the lives of their clansmen, children included. Toyotama decided to train her sons with their Sharingan, though they were not entirely ready for the advanced training. After deaths of Hashigoma's sons, she decided to settle the score with Toyotama. The Uchiha leader, however, did not back down from the challenge and welcomed it fully.

Toyotama rallies a small army to face with Hashigoma and her small army. While their respective armies fell, the two leaders remain to finish the fight, only for the battle itself to be drawn out into three days without rest. As the two leaders prepare to attack, they were halted by a mysterious woman who used a Chakra Rotation. Demanding to know what happened, Toyotama and Hashigoma both fell unconscious and were taken to a remote location. After a time of rest, Hashigoma awoke to discover a young woman named Higasa Hyuga who claims to be a being from another world much to her disbelief. As soon as Toyotama awoke, Hashigoma attempted to attack her, only for Higasa to stop her and have the two woman get along, much to their chagrin. While the Hyuga woman uses her Byakugan to heal the two rivals, Toyotama began pondering the situation they were in as well as Higasa's interference with their battle. Once healed, the two prepare for battle, but once again were halted by Higasa as she wished for them to stop this senseless fighting and urges them to get along. Hashirama and Toyotama decided to call a truce between the two for now and return to their respective clans.

Once she arrived, Toyotama received word that more members of her clan were killed, however she disregarded it as she stated, "its nothing new, we are expected to lose a few members." She knew that sacrificing the ones they love will awaken their Sharingan's power, something she was fully familiar with since she lost someone dear to her a long time ago. She decided to train her sons once more and that time she told them the secrets of the Mangekyo Sharingan, which can only be awoken when one experiences a great loss of someone precious to them. Before leaving, Toyotama privately tells Izuna of the Eternal Mangekyo Sharingan and one can only obtain it by taking another pair of Mangekyo Sharingan and if something were to happen to him or Madara, he was to give him his eyes. One night, Toyotama was visited by her acquaintance Higasa, as well as her rival Hashigoma, who asked for help on a certain matter. Unsure of this matter of which she spoke, she offers her assistance in the Hyuga woman's endeavor acting rather glad to take part, giving her rival a teasing look. After recruiting Toyotama, the three women set out on a journey that may decide their fate.

During their long trip, the three began to bond and learn more about each other as well as their respective lives. The three were able find some things in common with the other causing playful tension between them, more specifically between Toyotama and Hashigoma, leaving Higasa as the mediator. During this, they had suggested to train with each other in order to incorporate their skills when they encounter a rather difficult challenge. This training sparked into a competition between the women prompting them to laugh loud and full of joy, soon enough the three became friends. Their long journey came to an end when they had finally reached their destination as Higasa introduces them to the eight-headed snake demon known as Orochi. Shocked to discover that the "help" needed was to slay a powerful demon. The three legendary kunoichi began their battle against the demon, which had the women draw out their greatest powers. In the end, the three kunoichi exhausted the powers and abilities, though Orochi did not yield and prepared to finish them off, only for Toyotama to fully activate her Eternal Mangekyo Sharingan, activating her Complete Fullbody Susanoo, as well unleashing a forbidden technique known as Ryuhi-Otohime. The three kunoichi simultaneously launch their most powerful techniques against Orochi, completely overwhelming the snake demon and quickly destroying it. The battle was over and the legendary kunoichi were triumphant, however it came with a high price for using and exhausting their greatest powers. Toyotama thanked the two for being good friends and only wished for them to continue living and fighting alongside the other, bidding them farewell as she transformed into a dragon-like flame. Her death was never in vain as Hashigoma, Toyotama and Higasa made a difference; they not only saved a great part of the Ninja World from a demon, but they became friends rather than adversaries.

Toyotama was later reincarnated by Orochimaru, alongside Hashigoma and Higasa at a reincarnated Madoma's request. The three matriarchs were confused, yet glad to see one another again. Once Toyotama sees the last of her kin, Sasuke, she became overjoyed and hugs him with glee, only to confuse the young Uchiha. The three matriarchs proved to be far too powerful to be controlled, but were told what was happening on the battlefield and offered to assist. The Hokage, Senju siblings and matriarchs arrive on the battlefield to assist Naruto and the others in the battle against Madara, Obito, Black Zetsu, the Ten Tails and later Kaguya.
