It's not a hammer. A hammer would have flatter protrusions, for maximum injuries through bludgeoning the foe. I think it's a kind of axe or polearm. It's more than likely a weapon of Kaz's own design, as I can't find any weapons similar to it searching through the premodern weapons page of wikipedia. The closest thing I can find to it is the labrys, a kind of double-headed axe (en.wikipedia.org/wiki/Labrys ).
The chain on the end is most likely an addition by Kaz, as I can't find any polearm or axe-based weapons with a chain connected.
I don't know who Kyo is. I do have several theories. They don't seem to be a fusion of Abyss and Kaz, which is one thing I considered before changing my mind. The way they talk about Abyss makes it sound like they're separate from him. They COULD be the true identity of the Hooded Figure, but I'm extremely doubtful that Kaz will confirm or deny that until sometime after chapter 6. On Tumblr, she made a post saying she was concerned about giving away too many spoilers in the remake, and the Hooded Figure returns in chapter 2 and 6 in the form we see in Evolutiontale, in addition Abyss doesn't seem to know their identity or address them by the name Kyo.
It's also notable that their design has some similarities to the silhouette of Abyss' form once he gets all four 'Element Orbs', located here: Abyss sans four awaken part 4 final hide
A lot of differences as well, to be fair. But enough similarities that you should probably keep them in mind.
What's my personal opinion on this? I know who the Hooded Figure is not, but I don't know who they are. We know even less about Kyo. All we really know about them is they're connected to Evolution, and they bear similarities in their design to Abyss, Kaz, the Hooded Figure, and Obscuro, their life force may be connected to Abyss', and they do not share the same 'role' as Abyss. I have some suspicions but I'll wait for more information. Kaz surprised us with Kyo's appearance, and their true identity may be equally unexpected.
