It's not hard with the create-a-monster bodies, since the upper arms have the same simple peg-style attachment as the elbows and wrists. I just removed the arms, filled in the joints and built up the shoulder area on each side, and then made a small hole in each side, big enough to take the peg, but tight enough that the arm would hold poses.

I seal with Testors Dullcote or Mr. Super Clear before and after painting. I usually don't bother with priming before modding.

ModelAIR for some parts. I find it dries a bit harder than something like DAS, and much harder than paperclay. The rest is done with Apoxie Sculpt.

How well does the apoxie hold up? Would the doll remain durable enough for kids?

Apoxie is very durable and holds up extremely well, and I sculpt delicate features that stick out, like the ears and horns, over wire or toothpicks in order to reduce the likelihood of breakage. Depending on the kids, the mods might hold up, but the paint likely would chip with frequent play, particularly where joints rub together or in areas that are grabbed/touched a lot for posing.
Generally customized fashion dolls are recommended for adult collectors, though.

Because I'm thinking of modding hooves on a Rochelle to make some kind of steampunk white devil, but I'm not sure how

I cut them about 2/3 of the way through the plastic with wire cutters. Then I bent them and filled in the space with apoxie sculpt to hold the bend in place. Once that was fully hardened, I did the over sculpting.
