What horror film is this from? What is the meaning of the apple?

Thanks ~ it is inspired by several, the Classic Noh theater mask, The Ring, a couple different Youkai monsters (the tongue, the hair). The apple reference is from "Death Note" in which a death god loved apples. The apples were like a drug, and it has some sexual overtones as well.
