Dean was actually a smoker in the very first Supernatural script Kripke gave to the CW but the channel was afraid to have to deal with anti-tobacco associations (and a negative feedback in general) so they asked Kripke to step back when it came to his "smoking Dean" idea.

Hahaha, honestly I find that funny considering smoking is nothing compared to what they did decide to put in the show and how many other people they've angered (speaking as a religious person). I really loved the show myself but had to make myself stop out of guilt for all the things I was seeing (and also because I could feel the quality slipping after season 5). Either way I am not unhappy about their decision to dispense with the smoking idea, even though I have a lot of favorite characters who smoke. Dean's too much of a lost puppy to be smoking XD

It's a fan art, everybody is free to interpret their favorite character the way they want, and FOR ME, Dean is a smoker. And also, in the original script Dean WAS a smoker! 
And when it comes to destroying health....have you seen the life of the Winchesters? Smoking would be the least of their problems

1) it's a fan art, everything is allowed and I love seeing him smoking.
2) In the original script by Kripke Dean was a chain smoker. The idea was dropped just because the executives of the WB (now CW) were afraid of getting into trouble with anti-smoking associations.

a) it's a fan art, you can reinterpret the character the way you want. (and actually he was a heavy smoker in the original scrip but the WB, now-CW was afraid of criticisms so they stepped back)
