as far as the "cleanness" goes, i after i scan the image i give it a little tidy around the edges in photoshop to get rid of any palm smudging. Used to do this by hand with an eraser, but i accidentally creased a page once and ruined a pic, so now i do it digitally first
i dont change the subject at all in photoshop except for changing it to greyscale to get nicer blacks.
