The man's brain will be kept alive and conscious, hooked up to sensory apparatus so it may see, hear and talk. Locomotive apparatus is also an option. Wilmarth was invited to come to Yuggoth by some swaddled thing claiming to be Akeley. Wilmarth overheard some mumbled discourse below stairs and chickened out !

Oh, how deliciously GORY!
Setting aside the obvious gore factor...I think the lighting really sets the scene in this piece: Despite the medical table and the...uh..."surgeon" obviously working, the foreground lighting is dim instead of a flood of lighting as most examination tables are. This portrays a sense of secrecy and just makes things plain sinister. The near pitch-black of the background really adds to this; who knows what's watching from the shadows?
I really like the the monster/alien character's pose, its holding up the brain almost delicately which is a huge contrast on the brutal act of scooping out a human's skull.
Its a fantastic piece. I really like it, and not just for its gore either
