It is nice to find out a little bit more canon about Undertaker through a side story. ^-^ Now we know a little bit more about his scars.

I think that over time his body had been rotting off his bones, hence the skeleton grim reaper image we are all familiar with, so he takes the skin from some victims and stitches them on himself to appear more human

That is an interesting and creepy concept I don't want mental images of. XD
As interesting as the concept is and how it feeds into the image of reapers today, I don't think it really fits in with what Yana Toboso sees reapers to be, Their bodies are still strong and able to fuction like living people's do and we have seen no sign of the bodies rotting. And they do qualify as divine beings, so I could not imagine creatures working close to heaven being represented by zombie-like creatures.
Though, it is still is a pretty plausible and cool hypothesis.

Reapers could have evolved over time, and we know Undertaker is an OLD reaper-judged Robin Hood's soul in 1202, think of how long ago that was! As well, he COULD have gotten infections or the Plague, and there was no advanced medicines then which could have Β caused some of his body to rot. Syphilis back in Italy actually rotted peoples' skin off while they were still alive! Now its just spots on hands..
or maybe the skin had split open to reveal everything underneath and that is why he is stitched up because otherwise we'd get a very gory sight e.eΒ
OR maybe he just donned the ribcage and the skull Β of his first reaping and after the Dark Ages he just decorated his scythe with them and tied them together with those thorns..
