Sans' background is dark blue because of the color of your soul during his battle. It also looks like the sky with stars because Sans is the only one who might know how the stars look like on the outside, since you can reset the game after pacifist ending, and also because Papyrus says that Sans loves the space. It shows Sans sad holding Papyrus' scarf after his death, and on the back his friends that he lost several times. Also, Dr. Gaster is there for there seems to be some kind of relation between him and Sans. This all represents Capricorn's ability to hide their emotions and deal with frustrations, keeping calm and silent even when in suffering.
Sorry about the gigantic description on the picture, Sans is complicated.
