*stares at picture for a few minutes* The tattoo is a stylized bat, which I would guess would represent either Akito or Alex. For romance sake, let's say it represents Alex. Tattoos are permanent marks you consentually put on your body, and she has taken permanent residence in your heart, thus the position of the tattoo. It may hurt sometimes, but in the end, she won't disappear from her fixed place in your heart, no matter how much you yourself try to cut her from your life. Why you would do this, who knows? Perhaps fear of being hurt.
Hell, the same reasonings could be used for Akito, as well. Except in his case, you really can't cut him from your life because he's a part of who you are.
