Well, It all started with "Meet the Spy"
When Spy first said "Gentlemen" and began to use a cigarette. Someone on the web decided to make a joke of it, which resulted in an image of the Spy with multiple cigarettes in his mouth at once. Also, he was cross-eyed in the picture. Eventually, others on the web began to parody the image, using spy in other forms, then the other classes of Team Fortress 2, then soon other media, thus the image seen above. Not sure if I can say anymore specifics.
