Something's wrong with her mouth. Looks a little like it's not her or like she has broken jaws.

I really like the emotion captured in this, but for some reason the chain seems out of place. Maybe if you added some noise it'd make it look more - rustic?
Also, if the eyes were crimson and the thinger in her left hand had more sharpness or a clear identity, it'd give the image a hella eerie tone.

Doesn't look to realistic, the chains are way too bright for a dark setting like that and the mouth looks like it came from a game render.
By the way, the right arm is going through a bar and the left hands muscle under where the thumb would be is erased oddly.

It's depth man, depth. The right arm is in front of the bar. And the left arms muscle is too dark to see.
