Naruto tells Sasuke he loves him; Sasuke doesn't react well.

It's my headcanon that Sasuke can't forgive himself for what happens between himself and Itachi, which is why he suddenly becomes reckless with his life during his pursuit of revenge. He's so mortified by his own actions that he believes there's nothing left for him but to be a vessel for hatred; he thinks he's the scum of the earth, and as such, all he's got left to do with his life is carry out that role until it kills him. He stops caring about himself, and doesn't believe anybody else can truly care about him, either... because, he thinks, how is it possible to love the embodiment of hatred? How is it possible to love someone who's sunk as low as him?

If Naruto told Sasuke he loved him, Sasuke wouldn't want to hear it. He'd get angry and defensive. And no matter what his own feelings were for Naruto, he'd push them aside, because he believes he doesn't deserve any type of love- and along with that, he believes nobody deserves the attention or affection of someone as horrid as himself. He hates himself so much that he thinks he doesn't even deserve to feel love towards anyone.

...Of course, it's also my headcanon that Naruto gets through to Sasuke eventually, and helps him slowly begin to heal from his ordeals.
