The dark nature of both the image and the subject matter make for a good HDR image. The emphasis of the veins, it seems to me, is an excellent metaphor. His arm is like a roadmap, with the veins being the many pathways that we can choose between. Those roads, however, all lead to one place in the end as does are journey through life.
Whilst the end is as certain for all of us, it is the journey and the manner of that trip (sorry to use a pun) that is important

I had friends who were into drugs, and soon they had me smoking, then i got busted by the schools, and I got sent to alternative after all my "friends" threw me under the metaphorical bus.
needles to say, I am clean now, and away from that circle or "friends"

what a good shot that really shows the reality within the beauty. The beauty to me is the composition, how everything in the shot draws my eye in a timely fashion. It gets the point across, maintains privacy for the user, and shows the dirt and grit associated with the narcotic. Plus the venation in the guys arm is something else entirely. Is this shot in HDR?
