The cover, designed by Ray Lowry, features a photograph of Paul Simonon smashing his Fender Precision Bass against the stage at The Palladium in New York City on 21 September 1979 during the "Clash Take the Fifth" U.S. tour. At the time, The Palladium was the Mecca of punk and new wave music.

The photo was taken by Pennie Smith. It was later voted the best rock and roll photograph of all time by Q magazine, although at the time Smith did not want the picture used. She did not feel it was a technically good shot because the photograph is slightly out of focus (she was backing away from Simonon to avoid getting hit). However, Joe Strummer convinced her to use the shot. The broken remains of Simonon's bass are in the permanent collection of the Rock and Roll Hall of Fame in Cleveland, Ohio.

Simonon's picture is surrounded by pink and green typography similar to that on Elvis Presley's debut album. Through this artwork, Lowry tried to make a connection with Elvis, the Beatles and the flame-carriers of rock and roll. It has become one of the most recognized album covers of all time.
