Simplest method is to use IconBuilder, Plug-in of PSD. [Link]
or Convert file in Icon Composer of apple Xcode. [Link]

Drop "Localized.rsrc" file in
/iTunes.app/Contents/Resources/English.lproj/

And replace "Localized.rsrc" file.
