After losing most of his friends, Elixir was depressed. About to take his own life, Elixir was visited by Apocalypse who promised to take his pain in return for being his Horseman. Elixir accepted seeing no other alternative.

Recreated in Apocalypse's image, Elixir was now Famine. Using his abilities to hasten their metabolisms, Elixir's victim begin to waste away, literally starving to death. On the other hand, Elixir acts as the healer of the Horsemen and Apocalypse.
