No, she didn't. But Lonblaze really didn't mean to, she ws just too old. In "Night Whispers" they showed the elders of ShadowClan burying her, and the elder named Ceaderheart said that she had been his mentor!

Yea, but after Blackstar's reaction after what happened to her (Don't want to spoil anything) it's not hard to guess that they are mates.
