I haven't finished the short story, but I am familiar with Cthulhu. So what does the tentacle comeing out of your mouth represent? The fact that Cthulhu's face was like that, or something more?

The tentacles coming out of the mouth is representative of the moment you go insane upon setting eyes on Cthulhu.
I found the concept of that very interesting, so decided to capture it in a photo.

I think the idea is very cool, I do like it a lot. And it is quite creepy too.
The make up is cool too.
I think you could enhance the image though, I would have zoomed in a little closer so you don't see any of the background, because it doesn't really fit.
There is a little too much light, not using with flash could help, or atleast have a diffuser (those aren't too expensive).
Adding a texture can also make it a lot creepier.
