Thanks!  Relative distance?  Idk... can be however far you want!  I always imagined this set up as a way to understand the relationship of the planes, not a literal physically representation of where they are.  


I've copied some text below from a document I made for my players a while ago.  Haven't looked at it since 2007, haha!  As you might be able to tell, I borrowed freely from cosmology stuff from WoTC.  The format is basically this:


Name of plane (other names it goes by): deity that makes its home their, other pertinent notes.

The Material Plane: Contains the realm of Andaria

 

The Transitive Planes:

The Astral Plane (The Silvery Void, The Space Between): Transitive plane that contains all planar realities.

Shadra'han: (Plane of Shadow, The Toxic Dark, Lighthate, Darklands): Transitive plane that is both coterminous and coexistent with the Material Plane.  The King of Shadows dwells here.

Tal'Lunari (Ethereal Plane, The Misty Curtain): Transitive plane that coexists with the Material plane and often other planes (as they pass through). The Master of the Mind (psionic deity) dwells here.

The Nether (The Great Nothing): A paradoxical nothingness that exists outside the Astral.

 

The Outer Planes:

Acheron (The Battleground, The Bastion of War): The Bringer of Battle

Avenkatesh (The Eternal Day, The Crystalline Night, Heavenly Firmament): The Keeper of the Firmament

Benneth (The Sea of Flame, Everfire): The Flamemaker

Carceri (The Great Prison, Exile): The indefinitely remote plane of incarceration.  Thought to be home to the vestiges of pact magic.

Crestinnet (The Tower of Creation): The Builder

Crydon (The Plain of Ice, Glacier’s Rest, Chilling Death): The Icemaiden

Dzadalel (Realm of the Dead, Lel’s Bosom, Deathgates, The Dark): Lel the Deathmistress

Elidian (The Fortress of Knowledge): The Spellweaver

Farice (Home): The Hearthtender

Gabella'osenne (The Embrace, The Interminable Kiss): The Lover

Haania (Inspiration’s Home, The Perpetual Stage): The Mummer

H'raal (The Far Realm, The Outside, Realm of Madness): An indefinitely remote plane of utter madness and cyclopean insanity

Karasuthra (The Twilight Wood, The Mountain of Eternity): The Dweller in the Wild

Kayrana (The Azure Sky, Long Sigh, Ever-blowing Winds, The Windswept Depths): The Mother of Winds

Kerivteld (Earth’s Heart, The Heart Stone, The Blessed Fields): The Earthbreaker

Kersirenial (The Fey Court, Elfhome): The indefinitely remote plane of the extinct elves.  The elven deity Corellon once dwelt here.

Mal-telormar (The Soul Forge, The True Forge, Everforge, Mighty Anvil): Moradin and the dwarven hero deities

Nessus (The Hells): Orbits Dzadalel, home to Lel’s handmaiden Daamar

Oinis (The Gray Death, Bleak Eternity): Orbits Dzadalel, home to Lel’s handmaiden the Patient One

Sidrith (The Seething Malice): Orbits Dzadalel, home to Lel’s handmaiden Mila the Scourge

Sorthrollin (Forever Roads, The Long Road, Journey’s End): The Lady of Ways

Telisea (The Eternal Sea, The Forever Ocean): The Father of Waves

Tel Morwin (Region of Dreams, Dream): The indefinitely remote plane thought to be where dreams live.
