Heh this really took forever hehe
A4 size drawing, sketched first in pencil, then shaded mostly using stippling (dots) with fine black drawing pen.

Nah, just pen on paper. Mostly stippled with a very fine pen.

I'm sorry this is a very late reply but I've only just realised how badly I phrased that, I probably read the question wrong. 
They were fine nib drawing pens (.01mm), the make was probably Pilot I think for this. So, sort of similar to rapidograph, but not as nice.

Don't worry
I have got something similiar -> Faber Castell marker, O,1 mm, 0,3 mm and 0,8mm. But I've never thought about use this markers on this method
